# What Health Care Costs > Health care prices in the United States, read from the machine-readable files that insurers (Transparency in Coverage rule) and hospitals (Hospital Price Transparency rule) must publish. Insurer prices are summarised by state; hospital prices are shown hospital by hospital. The pages below are markdown. Each insurer page lists its procedures and links one table per procedure (`statemap__US.md` beside it): providers, lowest, median and highest negotiated price in every state, and the median as a multiple of Medicare. Each hospital page lists a state's procedures with the range of commercial prices across its hospitals.
