Doctor’s fee or facility fee? These are doctors' fees for a visit in their own office. An independent practice usually charges nothing more, but a clinic owned by a hospital may add a separate facility fee — worth asking before you book.
Who is counted: Florida Blue mostly lists practices, not individual clinicians: 70% of the 8,112 provider IDs (NPIs) it names belong to a practice, clinic or hospital, which stands for everyone who works there. So “providers” here mostly counts practices — one entry can cover many doctors — and is not comparable with insurers that list each clinician. Florida Blue’s files name 4,684 providers for this procedure; 1,602 are counted here: the clinicians whose specialty performs it — mostly Durable Medical Equipment & Medical Supplies, Clinic/Center and Nurse Practitioner — and the practices, clinics and hospitals. Insurers attach their whole fee schedule to every contracted provider, so the rest are clinicians listed at a price for something they never do; counting them would inflate the provider count and pull the typical price towards the fee schedule’s floor.
Not on this map: Care outside its home state: Blue Cross Blue Shield plans share one national network, BlueCard, for members treated in other states. Those prices are published centrally, not by Florida Blue, so they are not included here.
