Doctor’s fee or facility fee? These are doctors' fees for a visit in their own office. An independent practice usually charges nothing more, but a clinic owned by a hospital may add a separate facility fee — worth asking before you book. Where a state has both, the table and each state’s popup show the doctor’s fee and the facility fee side by side; their sum is a rough total for the visit, before anything else done during it.
Who is counted: Florida Blue mostly lists practices, not individual clinicians: 70% of the 8,112 provider IDs (NPIs) it names belong to a practice, clinic or hospital, which stands for everyone who works there. So “providers” here mostly counts practices — one entry can cover many doctors — and is not comparable with insurers that list each clinician.
Not on this map: Care outside its home state: Blue Cross Blue Shield plans share one national network, BlueCard, for members treated in other states. Those prices are published centrally, not by Florida Blue, so they are not included here.
