Prices come from the machine-readable standard-charge files each hospital must publish (45 CFR 180), dated 2026-03-31 to 2026-04-01. A negotiated rate is what an insurer pays the hospital, not what you owe: your deductible, copay and coinsurance decide that. "Typical" is the median across insurers. The cash price is the hospital's discounted price for paying without insurance; the gross charge is its list price. Some hospitals state part of their insurer pricing only as a percentage of charges or a formula; those rates have no dollar figure and are not shown. A grey ring is a hospital with no price for this procedure; click it to see why. Maryland is different: its Health Services Cost Review Commission sets each hospital’s rates, and every insurer pays the same rate (Maryland Health-General § 19-219). Where a hospital publishes that one rate, it is shown as "All insurers". Newer code, in use since October 1, 2023, when it replaced codes 246 to 249.
